* Note for physicals: All physicals should be completed on the current up-to-date UIL Pre-participation Medical History and Physical Form.(revised 2024)

• A new physical exam must be given prior to each school-calendar year, and has to be dated on or after April 1st for the new school year. Once completed, you may email the physical/history in pdf format to [email protected] .
• All students must have a physical, medical history, and all electronic forms completed prior to participating in an athletic class, practice, or event.
• 6th grade students DO NOT need an athletic physical/history. Prel-athletics require one electronic form on rank one.

Electronic Online Forms - (Signatures required by student and parent)

* To access the online forms hold your cursor over the “Electronic Participation Forms” tab. You will get a drop down list of the forms.

*Click on the form name and fill out the information requested.

* To sign the document click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box. After completing all forms, you will receive a confirmation email that they have been submitted. Forms DO NOT need to be printed.

* Note for physicals: Students are required to use the Pre-participation - Physical Examination Form provided. NO OTHER Physical Examination Form can be accepted. A new physical exam must be given prior to each school-calendar year, and has to be dated on or after April 1st for the new school year.
